Popular Skateboarder Bam Margerahas gone through a lot since he rocketed to stardom in the early 2000s in the MTV reality stunt show "Jackass" and its sequels "Viva La Bam," "Bam's Unholy Union," "Bam's World Domination," and "Bam's Bad Ass" Game Show.

He's suffered from alcohol and substance abuse since then, worsened by the death of his friend and fellow stunt performer Ryan Dunn in 2011. As a result, he has spent time in recovery facilities to get back on track.

However, such stints appear to have had little impact, as the actor and stunt performer was recently declared missing from a facility where he was housed, his second disappearance in two weeks.

According to a report obtained by Page Six from Florida local officials, the "Viva La Bam" star has gone missing after fleeing his rehab clinic where he was undergoing addiction treatment.

The report revealed that the pro skateboarder was last seen around the LifeSkills residential facility in Deerfield Beach, Fla., on Saturday around 5:30 p.m. At the time, he was dressed in "a black shirt, black trousers, and black sneakers."

Margera's whereabouts were investigated after he was not seen in his room when the orderlies checked him repeatedly. Once it was clear that he was no longer within the facility nor close by, the rehab's management reported the incident to the police as a missing person case.

Margera's recent escape from the treatment center is the second time he has been reported missing in recent weeks. He was first reported missing when he left a separate treatment center in Delray Beach, Fla., without permission on June 13.

At the time, the former MTV star apparently complained to the facility management that he was displeased with the facility's services and was checking out. Margera did not transfer to another institution to complete his therapy, remaining silent and refusing to give information about his whereabouts.

On June 15, he was located safe but lying low in a hotel room in Florida. Sources on the ground revealed that the authorities found him sober and calm and promptly sent him to a second treatment center, where he stayed for about a week before attempting his newest escape.

Bam Margera's Substance Abuse Issues

Margera's rehab visits began after seeking therapy for his substance addiction issues for many years. His first stay in such facilities occurred in December 2009, followed by more stints throughout the years.

He celebrated one year of drug and alcohol addiction therapy in May, according to TMZ. Margera told the publication that after the ordeal, he opted to relocate to a treatment center in Florida to be closer to his wife and child.

Margera was active on social media before he escaped from LifeSkills. On Friday, he shared an Instagram post with a throwback photo of himself and a message about his mother, April, that spoke about her "scrapbooking" for fun.

Following reports that he had gone AWOL from the treatment clinic, the TV personality also uploaded flashback photographs on Instagram of tattoos he had acquired from Kat Von D.

A Heavy Cost

Since his substance misuse forced him to check into a facility, the 42-year-old has suffered many setbacks. Margera lost his role in the most recent Jackass picture, "Jackass Forever," bringing his long-running involvement in prior installments to an unfortunate end.

The actor allegedly tested positive for Adderall at the time of his departure, prompting the showrunners to remove him from the production. Margera later filed a wrongful termination complaint, saying that the team was "well aware" of the medication and that he used it to manage his attention deficit disorder.

Margera resolved his controversial wrongful termination case against the "Jackass" showrunners in April this year. The "Bam's Unholy Union" star allegedly reached a secret deal with producers Johnny Knoxville, Spike Jonze, Jeffrey Tremaine, Paramount Pictures, MTV, Dickhouse Entertainment, and Gorilla Flic.
