"I think anyone who knows me knows the relationship I have and have always had with my, 'Gram,'" the actress wrote in the caption, along with pictures and videos of her grandma.

"Although inevitable, it would nevertheless be one of the hardest things I would ever have to go through the day she’d pass," she continued.

Mitchell also noted that her Gram "was and will forever be my best friend. From the imaginary games she would play with me when I was little to picking me up from high school because I was being bullied, to keeping scrapbooks of every article and magazine cover I have ever been in, she was my # 1 cheerleader," adding, "My heart is broken."

Sharing Her Grief

The "You" star noted that while she feels "so grateful to have been able to spend such incredible moments with her and my family over 34 years plus having Atlas also be able to have her own special relationship with her, it’s hard to say that this will ever get easier because right now the pain I feel is excruciating knowing that she is gone."

Mitchell admitted that she haven't picked up the phone yet to call her close pals to let them know of her grandma's passing.

"A part of me wants to pretend like everything is the same and that I’ve just gone a few days without FaceTiming her. The reality is too much for me right now," she explained.

"Because I have always shared her with the world I felt like I also needed to share the experience I am going through now which is trying to celebrate her life and the memories we have had with smiles in between the grieving," Mitchell shared on why she's showing her vulnerable side on social media.

A Message To Her Followers & to Her Granny

The actress also reminded her followers to call their loved ones before it's too late.

"If you’re so lucky to still have your grandparents around, do me a favor and call them just to say hi, because you never know when it will be the last time. ❤️" she noted and concluded her message by writing, "I love you Gram. And I know you’re looking down on me with a drink in hand."

Their Close Relationship

Mitchell often shows her off "Gram" on her Instagram account -- be it on a video call or when she's back home in Canada.

In March, she uploaded a video of her granny dancing with her and captioned the share with: "Haven’t seen my gram in over a year - all she wanted to do was dance ❤️"

The clip amassed over 900,000 likes as of today, in addition to 2,800 comments.

In an interview with ABC Audio, Mitchell spoke about the special bond her grandma has with her daughter Atlas Noa, 2, whom she shares with longtime boyfriend Matte Babel, 41.

"Even when Atlas was a baby, she gravitated towards my grandma differently than she did with anybody else. It’s crazy," she said. "I mean, I'm like, I see them and it brings tears to my eyes automatically because Atlas just goes to her."
